Incredibly patient and knowledgeable, our team is committed to helping patients feel better and be well-informed when making important health decisions. We look forward to learning more about you and your concerns.  Follow the steps below to ensure you receive the best care possible.

Patient Portal

You will want to assure that you have completed your check-in through our patient portal.  This will deliver a complete history of your medical care to our team.  Be sure to have a list of your medications with dosages.  Entering this information prior to your visit will allow us more time to spend discussing your health.

Payment Options

In order to file your insurance on your behalf, we must have your current insurance cards on file at the time of your visit.  Upon arrival we will scan your cards and verify your eligibility.  To protect you and your identification, you will want to bring a driver's license or other photo ID.  Failure to provide accurate information and eligibility may result in payment in full at the time of your visit.  Should you not have insurance coverage, we will as for your balance in full at the time of your visit.  Should your insurance require a referral, we ask that you have your primary care physician's office send this prior to your appointment.

Should you need to cancel or reschedule

Well prepared plans can sometimes be interrupted.  We ask for 24 hour notice should you need to cancel or reschedule.  This allows us to accommodate other individuals who may have immediate concerns.
